Open Postions at New England Hospice 

NEW ENGLAND HOSPICE is searching for a Social Worker for our Western Massachusetts team centered around Northampton.

We are a smaller family owned and operated hospice. For nearly 20 years New England Hospice has been serving patients and families facing serious illnesses and providing support to those who have lost loved ones. We maintain that hospice is about life and the quality of time we have left. Our mission is to provide a Superior Hospice Experience to all who need and seek our help regardless of their ability to pay or their complexity of care. Much of our care is provided in long term care facilities and assisted living settings in addition to private residences. Our teams include Hospice physician, nursing, social work, chaplain, hospice aides, and volunteers

Responsibilities include (but are not limited to):
  • Provides emotional support and counseling to patients and families from intake through bereavement;
  • Performs initial assessment and develops social work plan of care;
  • Presents patient info and goals in IDG meetings;
  • Assures physician orders are in place for services provided;
  • Makes referral to community resources as needed;
  • Assures coordination of treatment;
  • Informs families of the benefits they are entitled and services required to care for their loved one;
  • Accurate and timely documentation in accordance with agency policies and procedures
 
Experience: Prefer one-year social work experience in a health care setting. Experience with geriatric, oncology or hospice patients is desirable

Education: Licensed Certified Social Worker with an MSW from a school of social work accredited by the council on Social Work Education or Licensed Independent Clinical Social Worker
